Until recently \, complex care often was assumed to be a complicated form of healthcare de livered in a highly controlled\, time-limited setting. Examples might inc lude a multidisciplinary surgical team performing a transplant in the OR or care rendered to a critically ill patient in the ICU requiring consultativ e input from multiple medical specialties. Attention to the spiraling cost s of healthcare with focus on segments of the population that remain unwell despite clinical pathways to improve lead some to recognize that other\, c linical and nonclinical factors\, impact outcomes. Mental health\, substan ce use disorders\, past life experiences\, and social determinants of healt h substantially determine the pathway to health and well being. While such care is inherently complex\, seemingly impossible progress can occur with appropriate insight and approach. The 2022 Larry Lewis Symposium will addr ess complex care\, with Rachel Davis\, recent Director of Complex Care for the Center for Health care strategies\, as keynote.
Je sse Gubb is a Senior Research Manager at J-PAL North America\, a r esearch center that works to ensure that policy is informed by scientific e vidence. He leads teams that implement randomized evaluations\, develop res earch resources\, and provide capacity building to the research community. With Amy Finkelstein and others\, Jesse is currently conducting follow up a nalysis on the Health Care Hotspotting evaluation\, looking at the effects of the Camden Coalition of Healthcare Provider’s care transition model on o utpatient care. Jesse holds a PhD in political science from Harvard Univers ity.
Carter Wilson joined the Camden Coalition in 2013 an d serves as the Director of the National Center for Complex Health and Soci al Needs and Technical Assistance. Carter leverages his experience implemen ting programs in the City of Camden to lead national field-building efforts \, including the development and activation of core competencies for comple x care\, and oversees the technical assistance work with programs throughou t the country.
Prior to his current role\, C arter served as the Associate Director for Clinical Redesign Initiatives wh ere he oversaw operations and directed quality improvement activities for t he nationally recognized Camden City Medicaid Accountable Care Organization . Carter worked with the region’s health systems\, payers\, and primary car e community to pilot and scale interventions serving medically and socially complex patients\, including launching CMS’s Accountable Health Communitie s pilot to systematically screen patients in the region’s emergency rooms f or social determinants and provide navigation to needed services.
Before coming to the Camden Coalition of Healthcare P roviders\, Carter worked for the United States House of Representatives For eign Affairs Subcommittee on Africa\, Global Health\, and Human Rights. Car ter received a Bachelor of Economics from Old Dominion University\, a Maste r of Commerce and Economics from the University of New South Wales\, and a Graduate Certificate in Applied Statistics from the Pennsylvania State Univ ersity.

ndy Jotte MD is an emergency physician at Washington University in
St. Louis. He also serves as a Physician Advisor to Barnes-Jewish Hospita
l and a physician consultant to BJC Behavioral Health. During the years of
his clinical care in the emergency department his awareness of the complex
ities of needs of particular subpopulations of patients has grown\, leading
to efforts to address these determinants of health though his roles with t
he Familiar Faces\, Complex Care\, and Hospital-to-Housing programs at Wash
ington University/Barnes-Jewish Hospital. He serves as the Medical Directo
r for the Hospital-to-Housing program.
Calencia Mitchell is a manager in BJC Healthcare’s Community Health Improvement department. She helped led the d esign and implementation of the Hospital-to-Housing pilot program. Calencia holds a MBA and MHA from the University of Houston – Clear Lake. Within he r role at BJC\, she has worked the last 6 years to project manage various i nitiatives to support vulnerable populations\, including but not limited to complex care patients. The mission of BJC’s Community Health Improvement d epartment is to advance health equity for those most impacted by health dis parities in the urban and rural communities that BJC serves through authent ic and equitable partnerships with community stakeholders and community mem bers. Hospital-to-Housing is one of those initiatives.
Jonathan Belcher is a Senior D
irector of Programs at St. Patrick Center in downtown St. Louis\, where he
oversees fourteen different Long-Term Transformation programs. His programs
support individuals and families that come to St. Patrick Center with the
highest need. Since joining St. Patrick Center in 2009 he has helped thousa
nds of individuals and families transform their lives by finding permanent
housing\, behavioral health and other support services. Jonathan has been
working with Hospital to Housing and Healthy Housing since program implanta
tion. As a strong advocate for consumer directed care\, Jonathan created
St. Patrick Center’s consumer advisory board\, a place where clients have a
voice and also sits on the St. Louis City Continuum of Care Board. Jonatha
n graduated from St. Louis University with a Masters of Social Work in 2015
and received his Bachelors of Social Work from the University of Missouri
St. Louis in 2009.Madelyn Thiel is a Senior Financial Analyst in the Business Decision Support Department at Barnes-Jewish Hospital in St. Louis. Her role with Hospital-to-Housing Pro gram is to validate the financial sustainability of the program by analyzin g the benefit of the program to hospital operating results. In her role as an analyst at BJH\, she works collaboratively with Washington University Sc hool of Medicine on various strategic planning goals and special projects t o continuously improve practices\, utilization\, and performance. Madelyn received a Bachelor of Science in Economics and Finance from Southern Illin ois University and is currently enrolled at Webster University as a Master of Business Administration candidate.
